GUILD COMMUNITY SUPPORT PROGRAM
When you have schizophrenia, bipolar disorder, or another type of serious
mental illness, getting the right kind of help at the right time is an
important key to recovery. Loneliness and isolation often accompanies
mental illness
Individuals will find a wide
range of opportunities to get connected with people, services and activities
that meet their specific needs and interests. Guild Community Support
Program (GCSP) assures the full array of psychiatric rehabilitation services
essential to support recovery. Once connected with GCSP, individuals can
use one service or be involved in many.
Basic services help individuals:
- pursue education and employment
- participate in social and recreational opportunities
- solve housing issues
- apply for and keep benefits
- prevent crisis
- connect with peer support resources
Some individuals
may be eligible for more intensive services, including community treatment,
medication assistance, and/or “hands-on” help to meet basic
living needs such as housekeeping, food preparation, shopping, plus case
management / service coordination activities. A multi-disciplinary team,
including case managers, nurses, employment specialists and psychiatry,
provides these services when and where needed. As an individual’s
needs change, services increase or decrease. Flexible, strengths-based
services help prevent the loss of housing and employment. Community alternatives
to hospitalization are used whenever possible.
Features include:
- “Leisure Partners” as a means for finding people with
similar interests.
- Building and sustaining personal support systems.
- Targeting services to help those who have the greatest difficulty
with basic daily activities and who have persistent symptoms that are
not easily relieved.
- Taking a collaborative, strengths-based approach to foster a spirit
of hopefulness and belief in each person’s ability to grow and
change.
Eligibility Criteria:
Guild Community Support Program serves:
- adult residents of Dakota County (age 18 and over)
- who have a diagnosis of mental illness (eligibility for some services
is limited to those who have a serious and persistent mental illness)
- who are referred by Dakota County Social Services for case management
services

- is CARF accredited for the following services: Assertive
Community Treatment, Case Management, Crisis Stabilization, Intensive
Residential Treatment Services, Mental Health Rehabilitation, Supported
Living, and Community Employment Services including Job Development,
Job-Site Training, and Job Supports.
- is an enrolled provider with Minnesota Health Care Programs and
other health plans including Blue Cross Blue Shield, Health Partners,
United Behavioral Health, and UCare.
